  The Theology of the Body Education Workshop is scheduled for Saturday, February 18, 2023, from 10:15 AM to 1:00 PM in the Life Center. Please complete the registration information below before February 8, 2023. Many parent volunteers are also needed in order to make this a very special experience for our 6th-grade students. Please consider helping in one of the areas listed at end of this form. You will be contacted with specific details regarding the volunteer opportunity. There is a $10.00 registration fee which can be paid online at the bottom of this page. If you prefer not to pay online, you may make the payment at the February FFF session, or mail to the attention of Debbie Blackburn at St. Vincent de Paul Church, 1502 Wallen Rd., Fort Wayne, IN 46825.

  I will not hold the St. Vincent de Paul Parish, or St. Vincent de Paul Parish staff, volunteers, or chaperones associated with this gathering responsible in the event of injury. Further, I agree to accept any and all financial responsibility as a result of scheduling necessary emergency medical treatment.

  St. Vincent de Paul Parish and the Diocese of Fort Wayne – South Bend are hereby granted permission to publish photographs taken of my child during their attendance at the Theology of the Body Education Workshop. Photographs may be used in the parish bulletin, parish or school web site, school newsletter, or parish newsletter.
  
  By agreeing to this form, I grant permission for use of photographs and verify that I am the legal parent or guardian of the child named above. I understand that St. Vincent de Paul Parish will not use photographs of my child unless specifically authorized in accordance with the policies of the Diocese of Fort Wayne – South Bend.
