Camper Registration Form

I grant permission for my child/children named above to participate in the St. Vincent de Paul Catholic Kidz Camp held June 3 - 7 from 9:00 AM to Noon.

I will not hold the St. Vincent de Paul Parish, or St. Vincent de Paul Parish staff, volunteers, or chaperones associated with this gathering responsible in the event of injury. Further, I agree to accept any and all financial responsibility as a result of scheduling necessary emergency medical treatment.

I will not hold St. Vincent de Paul (Parish / School) responsible for any injury incurred on the trip to and from this gathering. I hereby warrant that to the best of my knowledge, my child is in good health, and I assume all responsibility for the health of my child.

In the event of emergency, I hereby give permission to transport my child to a hospital for emergency medical or surgical treatment. I wish to be advised prior to treatment by the hospital or doctor. 

The fee for Catholic Kidz Camp is $25. To pay online, click here before submitting this form. You will be taken to the online fee payment portal. After making your payment, return to the registration form to submit. If you prefer to mail your payment, it can be sent to the attention of Debbie Blackburn at St. Vincent de Paul Church, 1502 Wallen Road, Fort Wayne, IN 46825.
