A FOB for the Adoration chapel allows you to enter the chapel between the hours of 8 pm and 5 am, when the building is locked and not open to the general public. For the protection of all our adorers, we reserve the right to conduct a background check on any person who requests a FOB. If you would like to request a FOB, you must complete all portions of this form and provide authorization for St. Vincent de Paul to conduct a background check. If you do not provide the requested information and authorization for the background check, a FOB may not be issued.
